Problem with CCSLoad and ICD-U40

I am having a problem with CCSLoad Version 4.012.

I am running PCWH Version 4.093, and am using an ICD-U40 with firmware
version 2.62 and Hardware Rev 5.

Until yesterday I was using the old ICD program and everything worked
fine. I am working with a PIC16F767 at 5 Volts, using the internal
oscillator at 8 MHz.

I downloaded CCSLoad (and updated all software using CCSdownload), and now
I can't debug my programs within the IDE. When I try, the program
attaches and downloads, but when it tries to run I get an error "Could not
start target: the target was not halted after reset Check the target
Oscillator and MCLR. Maker sure the target clock match the clock fuse and
that the target is oscillating. Use view | Valid fuses to find the
correct clock fuse."

I can run CCSLoad by itself, and everything works fine - I can download
and run the code and it works, but I can't debug.

If I go to "Diagnostics" in CCSLoad, and try the "Test Oscillator and Debugger" button, I get the same error message as above.

I get all of the same symptoms using an external crystal at 4MHz.

I'm hoping that this a known problem with a known fix.
