How do I debug other compiler's output with the CCS ICD?

Has anyone figured out a way to debug the hex file generated by the Microchip C compiler with the CCS ICD?

I'm not worried about having all the nice bells and whistles. I'd settle for being able to set a breakpoint at an address and dump register values when I hit it.

I played around with generating my own .COD file from the microchip listing, but realized I didn't know enough about the differences between the ICD2 and CCS' ICD to get this tactic to work.
