ICD-S40 blew up

Has anyone out there ever damaged their ICD? I have the ICD-S40 and I was on the phone to tech support and that was our only conclusion. It won't program any of my evaluation boards (16F877A and 18F452). It almost programs my production board but when it does a verify, the LVP, NOLVP fuses don't match.

It is funny how the similar problems happen around the same time...Take a look at this, it might help:
