ICD-U40 Hookup questions?

I just got an ICD-U40, and need to resolve some ambiguous installation instructions.

The pinout I see shows +5V on ICD #5. Some places seem say the target board must supply +5 to the ICD, others seem to say that it doesn't.

What if I have a +3V target?

Is the target supposed to be powered when programming?

Any help would be appreciated,

I don't have a CCS ICD unit, and CCS doesn't post the manual
for it online. In this poster for the Microchip ICD2, on the
right side of the page, they describe how ICD2 can be used
with a +3v target. The question is then: Does the CCS unit
have a similar circuit ? What does the CCS ICD-U40 manual say ?

I got a floppy disk that I can't read, other than that I have no manual.
