I've eventually come across MPLAB's document DS51764C (Multi-Tool Design Advisory).
It's dated 2010, and I'm assuming still current.
Includes this paragraph.
MPLAB ICD 2 Unit Upgrade
If you are having problems with ICD operation, please check the
revision of your pod, found on the back of the unit. If the part number
is 10-00319 R15 through R21 without an “ECO 3013” sticker, please
contact your local Microchip FAE or sales office to return the unit for
replacement.
Unfortunately mine's outside the update range, I'll have to move to ICD3 or Pickit3.
I hope this nugget will be useful to someone.

Hay Mike
I'd contact Microchip anyway, give them the numbers, explain the situation. They may actually either upgrade your unit or send you a PICKit 3.
I got a newer PicstartPlus from them for free ! as after I upgraded mine, it still didn't perform right.
never, ever hurts to ask !!
